Strong Body Pilates Liability Waiver

I am voluntarily participating in Pilates. I have been to a medical professional and cleared for physical activity. I understand that there are risks associated with my participation in Pilates, such as physical and/or psychological injury, pain, suffering, illness, disfigurement, temporary or permanent disability, death or economic loss. These injuries or outcomes may arise from my own or other’s actions, inactions, or negligence, or the condition of the Pilates location (s) or facility (ies). Nonetheless, I assume all risks of my participation in Pilates, whether known or unknown to me, including travel to and from Pilates (including air travel) or any events incidental to Pilates. 

If, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately. 

I agree that I, my heirs, personal representatives and next of kin, legal representatives and assigns will not make claims against Pilates DNA, LLC, DBA Strong Body Pilates, herein referred to as Strong Body Pilates, for any injury or damage resulting from my participation in Pilates at Strong Body Pilates. I have carefully read this agreement and release of liability and fully understand its contents. I am aware that this is a release of liability and a contract between myself and Strong Body Pilates, and sign it of my own free will. 

If any legal dispute arises, the prevailing party shall be reimbursed reasonable attorney fees, costs, and expenses in addition to any other relief to which the party may be entitled. 

The parties agree to first try to resolve the dispute with the help of a mutually agreed-upon mediator in the city of Los Angeles. If it proves impossible to arrive at a mutually satisfactory solution through mediation, the parties agree to submit the dispute to a mutually agreed-upon arbitrator in the city of Los Angeles. Judgment upon the award rendered by the arbitrator may be entered in any court having jurisdiction to do so. This Agreement will be governed by the laws of the state of California.

Strong Body Pilates Client Policies

Scheduling: 

  • Lessons/classes are scheduled by appointment only. 
  • Online class sign-up closes 45 minutes prior to class. After that time please call. 
  • If no students sign up for class by the start time, the instructor leaves and there is no class.

 

Payments: 

  • We accept cash, check, Mastercard, Visa, American Express, and Discover. 
  • Because of our cancellation policy, we reserve your initial session/class on a credit card. 
  • All payments are non-refundable. There is a $25 charge on returned checks. 
  • No swapping of one type of class/session for any other type. 
  • Payment is due before your scheduled session/class, unless authorized otherwise by the front desk or your instructor. To ensure the time you prefer, scheduling ahead of time is recommended.

 

Expiration Date: 

If sessions/classes are not redeemed six months after purchase, they will be forfeited. 

24 Hour Cancellation Policy: 

We require at least 24 hours notice if you need to reschedule or cancel. This is to give us plenty of time to notify your instructor and/or to fill your spot. You will be charged full price for your session/class if you do not cancel or reschedule in time, or if you do not show up or call 15 minutes into your scheduled start time. The teacher assumes you are a “no show’ and reserves the right to give your spot to someone else or leave if you are the only student scheduled in a class. 

If you schedule a duet session and your partner reschedules or cancels in time, we will do our best to find another partner for you. You may opt to upgrade to a private lesson.
